NOVAMag^® SHIELD Technique with Immediate Implant and Immediate Temporization

A patient presented with a failing maxillary anterior tooth in the aesthetic zone associated with a missing buccal bone wall, representing a high-risk situation for immediate implant therapy. An atraumatic extraction followed by immediate implant placement was performed.

Due to the absence of the facial bone, the NOVAMag^® SHIELD was applied as a rigid protective barrier in a flapless minimally invasive approach, allowing controlled buccal contour augmentation using cerabone^® +HyA and preventing soft tissue collapse during healing. CTG from the palate was positioned and sutured in the pouch to enhance the buccal mucosal thickness.

Immediate temporization was used to support soft tissue shaping and preserve the emergence profile. After healing, stable peri-implant tissues and a preserved facial contour were achieved, resulting in a predictable and aesthetic final outcome.

THE CHALLENGE:

A patient presented with a failing maxillary anterior tooth located in the aesthetic zone, combined with the absence of the buccal bone wall. This situation represents a high-risk indication for immediate implant placement, as the lack of facial bone significantly increases the risk of soft tissue collapse and compromised aesthetic outcomes.

THE APPROACH:

Following an atraumatic extraction, an immediate implant was placed. Due to the missing buccal bone, the NOVAMag® SHIELD was used as a rigid protective barrier within a flapless, minimally invasive approach. This allowed for controlled buccal contour augmentation using cerabone® + HyA, while effectively preventing soft tissue collapse during the healing phase.
To further enhance soft tissue quality, a connective tissue graft (CTG) harvested from the palate was positioned and sutured into the buccal pouch, increasing mucosal thickness. Immediate temporization was applied to support soft tissue shaping and maintain the natural emergence profile.

THE RESULTS:

After the healing period, stable peri-implant tissues were observed with preservation of the facial contour. The combined use of the NOVAMag® SHIELD, soft tissue augmentation, and immediate temporization resulted in predictable tissue stability and an aesthetic, harmonious final outcome in a challenging clinical scenario.
